Office of Long-Term Living (OLTL)

ACT 150 · Community Health Choices (CHC) Waiver · OBRA Waiver

• In-Home & Community Support: Assistance and skill-building for independent living at home and in the community.

• Companion Services: Daily engagement and support for meaningful, connected living.

• Respite In-Home: Trusted in-home relief for family caregivers.

All services are delivered according to each individual’s ISP and align with ODP and OLTL service definitions. Where Values Matter.
